Diabetes Prevention Program (DPP)
The Diabetes Prevention Lifestyle Change Program (DPLCP) is a year-long program designed to help individuals at risk for Type 2 diabetes develop healthier lifestyle habits that can prevent or delay the onset of the disease.
Through group-based learning, goal setting, and personalized support, participants learn how to improve nutrition, increase physical activity, and build sustainable habits that support long-term health.
This program is completely free of charge, but it requires a strong commitment to personal growth, consistency, and participation.

Program Outcomes
Participants in the program work toward measurable health improvements, including:
Losing 5–7% of their starting body weight
Achieving at least 150 minutes of physical activity per week
Building healthy habits that support long-term wellness
Receiving guidance and accountability from a trained lifestyle coach
Participating in a supportive group environment with others working toward similar goals
Eligibility Requirements
Participants must meet all of the following criteria:
Be 18 years of age or older
Have a Body Mass Index (BMI) of 25 or higher
(23 or higher for Asian Americans)Not have been previously diagnosed with Type 1 or Type 2 diabetes
Not currently be pregnant
Participants must also meet at least one of the following conditions:
Blood test results within the prediabetes range within the past year
A previous diagnosis of gestational diabetes
A high-risk score (5 or higher) on the Prediabetes Risk Test
Prediabetes test ranges include:
Hemoglobin A1C: 5.7–6.4%
Fasting plasma glucose: 110–125 mg/dL
2-hour plasma glucose: 140–199 mg/dL
Program Schedule
The program runs for 12 months and is structured in two phases.
Months 1–6
Participants meet weekly to complete the core curriculum and develop foundational healthy habits.
Classes are held Saturdays at 10:00 a.m. and last approximately one hour.
Months 7–12
Participants meet monthly to reinforce the key principles learned earlier and maintain healthy lifestyle practices.
Classes are held on the first Saturday of each month at 10:00 a.m.
Summer Virtual Sessions
• Tuesdays: 5:00 – 7:00 p.m.
• Saturdays: 9:00 a.m. – 12:00 p.m.
Participation requires dedication and commitment, but the program provides valuable tools and support for improving long-term health.
